Scots Gap Medical Group is a well-established practice based in Scots Gap with a branch surgery at Stamfordham, we cover a large rural demographic with an increasing list size. We are looking for an experienced practice nurse to join our team.

Job Responsibilities
* Provide treatment and care in conjunction with the GPs or independently as appropriate and according to practice policy and guidelines
* Take responsibility for own clinics and time management
* Review and filing of own test results and those relating to chronic disease
* Advise patients on general health and minor ailments with referral to the GP as necessary
* Immunisations and vaccinations including childhood immunisations
* Holiday vaccinations and travel advice, in line with GP Contract
* Venepuncture
* First aid for presenting emergencies
* Diagnostic tests such as ECG, urinalysis, swabs, blood tests
* Responsible for nursing management of patients with chronic disease/long term conditions
* To initiate, develop and implement protocols in relation to patients with chronic disease
* To carry out cervical cytology
* Carry out new patient health checks, well man and well woman checks
* Actively promote health education and weight management advice
* Provide family planning and contraception advice
* Act as chaperone where required
* To promote and run health campaigns as appropriate
* Stock and immunisation stocks monitoring and upkeep
* Risk assessments in accordance with COSHH
* Maintain practice policies and procedures related to recording, drugs administration, child protection, confidentiality, health and safety
* Infection control adherence and up-to-date practice policies
* Computer literacy and accurate patient data entry
* Contribute to and oversee QOF targets relevant to the practice nurse role
* Support PCN initiatives when required
* Maintain disease registers
Relationship & Accountability
* Responsible to the Partners for clinical issues and the Practice Manager for administrative issues
* Work closely with team members and liaise with admin, reception staff, District Nurses, Health Visitors, Community Matrons, School Nurses
* Liaise with secondary care and social services to ensure safeguarding, quality and continuity of care
* Attend monthly multi-disciplinary meetings
* Maintain annual NMC registration and work within NMC guidelines
* Be accountable for own practice and maintain records per NMC guidelines
* Maintain CPD, re-validation and PDP as required
Qualifications
* Essential: RGN qualification; Current NMC registration; trained in chronic disease management (including CHD, diabetes, asthma, COPD); up-to-date training in childhood, adult and travel immunisation; excellent interpersonal and time management skills; self-motivated and resilient; knowledge of infection control procedures
* Desirable: Cervical cytology training; up-to-date contraception training including depo-provera; ability to administer injections (e.g., zoladex, degarelix); knowledge of EMIS Web; knowledge of primary care targets
Experience
* Desirable: Experience of working in primary care
