Here, you’ll collaborate with a respected national home health agency to provide high-quality, personalized care to patients in the comfort of their own homes.
Contract Details:
* Location: Dallas, GA and surrounding areas
* Company: Sunbelt Staffing
* Contract Length: 13 Weeks (with potential for extension upon successful completion)
* Hours: Full-time, 40 hours per week, Monday-Friday, 8am-5pm, with occasional weekends and on-call duty (typically one weekend per month)
* Compensation: Negotiable, based on experience and territory budget
* Mileage Reimbursement: $0.48 per mile from patient home to home
* Blended Pay Packages: Available for travelers
* Benefits: Medical, dental, vision, and 401k match after 90 days
* Productivity: 25-32 visits per week, up to 35 patients
Your Responsibilities:
* Conduct comprehensive patient assessments using OASIS, ensuring accurate documentation and compliance with agency standards.
* Develop and implement individualized care plans, coordinating with physicians, physical therapists, and other healthcare professionals.
* Administer medications, monitor vital signs, and provide skilled nursing care, including wound care, IV therapy, and catheter care.
* Educate patients and their families about care plans, medication management, and other health-related topics.
* Maintain accurate patient records in Homecare Homebase (HCHB), ensuring compliance with regulatory requirements.
* Travel to patient homes within the area using reliable transportation.
* Participate in interdisciplinary team meetings and case conferences to ensure coordinated and effective patient care.
* Adhere to Joint Commission standards and other regulatory requirements.
What We’re Looking For:
* Active Registered Nurse (RN) license in the state of Georgia.
* Minimum of 1 year of home health experience.
* Proficiency in OASIS and Homecare Homebase (HCHB) required.
* Oasis Certified strongly preferred.
* Current American Heart Association or American Red Cross Basic Life Support (BLS) certification.
* Reliable transportation with a valid driver's license.
* Ability to meet Joint Commission onboarding standards upon offer.
