Position: Young Adult Transition Coordinator
Location: The Prince & Princess of Wales Hospice, Bellahouston Park, Glasgow
Hours: 30hrs per week (4 days)
Salary: £40,685 to £47,334 (FTE £50,860 to £59,159)

The Prince & Princess of Wales Hospice is a well-established charity; we have recently moved in our new purpose built home located within Bellahouston Park. The Hospice provides specialist palliative care for people with a life-limiting and progressive illnesses and provide support for their families and carers.
We are seeking a Registered Practitioner to lead and coordinate the transition of young adults with palliative care needs from child services to adult hospice services. This pivotal role involves working across NHS Greater Glasgow and Clyde, The Prince & Princess of Wales Hospice, Children's Hospice Across Scotland (CHAS), collaborating with multi-disciplinary teams, education leads, social work, and national partners.
Key Responsibilities:
* Lead and support transition pathways for young adults with life-limiting conditions.
* Contribute to the delivery of holistic, specialist palliative care during transition periods.
* Collaborate with NHS and hospice teams to develop a national blueprint for transition.
* Promote partnership working with young adults and their families.
* Contribute to education, audit, research, and national strategy alignment.
* Collect and report data for the Transition Project in partnership with Hospice UK.
