Position: Interim Program Coordinator
Type: Temporary
Hours: 35 hours per week
Pay: $30.00 – $31.00 per hour (actual determined by experience and other factors permitted by law)
Supervision: Under the guidance of the Adult Team Services Program Manager, the coordinator assists with the organization and direction of Personal Services Coordinators and other assigned staff of the Adult Integrated Services.
Summary
Under the general supervision of the Program Manager, the Program Coordinator assists the multidisciplinary team in delivering services to program members, aiming to help individuals with mental illness live independently in their chosen community. The role provides leadership training, coordination, and facilitation of efficient delivery of services.

Essential Duties and Responsibilities
* Oversee and ensure accurate monthly, quarterly, and yearly data reporting.
* Assist with coordination of outreach, enrollments, transitions between teams, and disenrollment of program members.
* Support documentation standards of staff per MHALA, County Contracted Provider, and funding source requirements, ensuring 24‑hour turnaround documentation.
* Provide support to individuals on the team as directed by the Program Director or Program Manager.
* Support staff in linking members and clients to meet goals and needs.
* Participate in team, staff, and community meetings; assist with day‑to‑day operations and quality improvement initiatives.
* Engage in community life and provide timely feedback to the RC Team on program issues; provide emergency coverage as needed.
* Maintain a member list of five members and meet a billing goal of 50 hours per month, mentoring the RC Team PSCs per the Full Service Partnership guideline.
* Attend staff meetings and support day‑to‑day operations and community life, ensuring seamless delivery of services.
* Assist with documentation standards, ensuring all required data collection documents are submitted on or before due dates.
* Collaborate with members to establish a working budget for housing and basic needs.
* Mediates and advocates on behalf of members to obtain and keep housing and health care benefits.
* Perform collateral duties as directed by Program Manager and Program Director.
* Provide timely information to Program Manager and Program Director regarding emergent staff or member issues; work with executive management to intervene when objectives are not met.
* Visit members in their residences as needed to help them live successfully and prevent homelessness.
* Transport members to appointments and errands when appropriate.
* Participate in site management, quality improvement, and designated leadership teams.
* Monitor completion of recovery plans for each member.
* Read, research, attend classes, workshops, Relias trainings, or seminars and engage in professional development.
* Complete all required documentation (e.g., transitions, discharges, 3‑month, KECs, determinants, MORS, monthly reports, individual service plans) and maintain paperwork for program compliance.
Education & Experience Matrix
* High School Diploma or GED + minimum of 5 years’ experience providing psychosocial rehabilitation services to a disabled population plus supervisory/leadership experience.
* AA degree in a human service field + minimum 4 years’ experience providing employment‑related services or sales.
* Bachelor’s degree in a rehabilitation‑related field or marketing (e.g., rehabilitation counseling) + minimum 4 years’ experience providing employment‑related services to a disabled population or sales.

* Good basic English and writing skills, computer skills, and high organization.

* Valid Class C driver’s license, vehicle, insurance; eligible for MHALA liability insurance.
* TB clearance and annual re‑testing.
* COVID‑19 vaccination or booster requirements may change; non‑compliance may result in termination.
* Provide employment verification from three past employers within the last seven years.
