An exciting opportunity has arisen to join the Discharge to Assessmunity Team.
This role is specifically to cover Summer Hill Terrace, which is part of the homeless/housing pathway from hospital discharge.
Your new role
You will work as part of the Discharge to Assessmunity Social Work Team to deliver on person-centred practices where citizens are enabled to exercise choice, control, independence, and build resilience with a focus on prevention and early intervention to support, provide guidance and signposting in addition to assessments, via the 3 Conversations model, in accordance with the Care Act 2014. The cases can be quiteplex as the support in Summer Hill Terrace deals withplex housing issues, substance misuse issues, safeguarding issues and working in a very multidisciplinary setting.
What you'll need to succeed
1. Have a recognised professional Social Work qualification (MA/BA/ in Social Work, Diploma in Social Work or CQSW)
2. Social Work England registration
3. 1 year post-qualifying experience.
4. Previous experience working within Social Work adults services
5. Strongmunication and interpersonal skills.
6. Ability to work independently and as part of a team.
7. Flexibility to travel as required.
