Key Responsibilities
* Deliver compassionate care and set a strong example for colleagues.
* Lead advanced care planning with patients and families.
* Work closely with the wider PCN team to improve patient outcomes.
* Develop and implement best practice initiatives.
* Coordinate patient activity and ensure high standards of care.
* Ensure effective communication with patients, families, and the wider community.
* Participate in clinical governance, audit, and quality improvement activities.
Qualifications
* Experience in specialist palliative care, including complex needs and end‑of‑life care.
* Experience in advanced care planning and advanced care discussions.
* Registered nurse on the Nursing and Midwifery Council (adult registration) with adherence to professional standards.
* Ability to connect with patients and families emotionally.
* Demonstrated research, data collection, and case study development contributions.
* Commitment to full‑time hours Monday through Friday.
