Kingston & Richmond NHS Foundation Trust is recruiting five Consultants in Geriatric Medicine to shape the future of frailty care across south‑west London. The Trust has a proven track record of innovation in frailty and is nationally recognised for its Frailty Improvement Collaborative and London‑wide frailty network.
Job Summary
Join a forward‑thinking team at a Trust rated Outstanding by the CQC. Work across acute frailty units, Care of the Elderly wards, integrated Virtual Ward, and community rapid response services. You will design and deliver new models of care that cross organisational boundaries and help reduce avoidable hospital admissions.
Main Duties of the Job
* Consultant‑led ward care on dedicated Care of the Elderly wards.
* Front‑door Frailty SDEC service, including early supported discharge and admission avoidance.
* Integrated community frailty care via the Virtual Ward and rapid response MDTs.
* Develop specialist liaison roles (surgical, orthogeriatric, or other).
* Provide clinical leadership, shape new frailty pathways, and collaborate across primary, secondary, and community services.
* Active role in teaching, training, audit, and service improvement with the Trust’s quality improvement programme.
* Help build a seven‑day integrated model of care to improve outcomes for older people.

Job Responsibilities
* Provide high‑quality Geriatric Medicine across acute, community, and frailty pathways.
* Lead ward care on dedicated Care of the Elderly wards.
* Deliver front‑door Frailty SDEC service, including early supported discharge.
* Engage in integrated community frailty care via Virtual Ward and rapid response MDTs.

Person Specification
Essential Qualifications
* Full registration with GMC.
* MRCP (or equivalent).
* Specialist Register in Geriatric Medicine, and for GIM and/or Acute Medicine.
* CCT in Geriatric Medicine & G(I)M and/or Acute Medicine.
* Advanced Life Support Certificate.
* Demonstrable experience in research with an appropriate publication record.
* Behavioural and attitude alignment with Trust values.
Desirable Qualifications
* Higher degree (MD, PhD or equivalent).
* Ability to engage busy clinicians in the development of the frailty service.
* Ability to develop research projects and supervise trainees.
* Understanding of modern medical curricula and learning assessment tools.
Skills
* Senior decision maker in multidisciplinary teams.
* Effective staff and resource management.
* Excellent communication with GPs, community professionals, and patients.
* Teaching and supervision of medical students and residents.
Knowledge
