About the Role
As a Needs Assessor / Service Coordinator in the Disability services, you will work with individuals and families to identify and coordinate support as required. This role is office based however much of the work takes place in the wider community. You will be working with wide variety of people, from the individuals themselves, to providers, health practitioners, schools, and community organisations.
You will work with our team based in Richmond and have close working relationships with Nelson Hospital, other health agencies and community services. We will value your commitment and experience in working with people and their families to ensure that their needs are at the centre of their care.
Our aim is to support people whose lives are affected by a disability, to achieve their goals, retain their independence and self-reliance, and to enjoy optimum quality of life wherever they live.
This is a permanent full-time role, working 80 hours per fortnight (1.0 FTE), Monday to Friday 8.00AM to 4.30PM.

About you
As a Needs Assessor / Service Coordinator in the Disability services, you will:
* Be a motivated and enthusiastic individual, ideally with experience working with individuals with disabilities;
* Have knowledge and understanding of disability models of care and support desired;
* Hold a full driver’s licence.
We are looking for a person who:
* Has experience working with individuals with disabilities;
* Has the ability to work in both a team environment as well as independently;
* Is flexible and adaptable.
The successful candidate must be eligible to work in New Zealand or hold a valid NZ visa at the time of applying.
