Job Overview
Adult Assessment to determine competence in delivering services, promoting and empowering users. Offer information, advice and refer them as appropriate to external services to meet identified needs.
Key Responsibilities
* Experience of working with vulnerable adults through illness, disability or age-related disability
* Assess adult service users and carers
* Understand age-related illness, disability and envy and their impact on individuals, carers and family
* Apply relevant legislation such as Social Services and Well-being Act 2014, Mental Health (Wales) Measures 2010, Mental Capacity Act 2005 and Mental Health Act 1983
* Understand safeguarding issues and procedures in relation to adults
* Maintain a good general standard of education
* Register with Social Care Wales
Qualifications
* A recognised degree/diploma or equivalent qualification in Social Work (e.g. Degree, DipSW, CQSW)
* Registering with Social Care Wales

About the Service
The Vale of Glamorgan Community Older People’s Mental Health Service is an integrated team made up of local authority and health professionals with specialist knowledge of mental disorders. Professionals in our team include Social Workers, Approved Mental Health Professionals, Psychologists, Psychiatric nurses, Occupational Therapists, Physiotherapists and Psychiatrists. We hold a weekly meeting that gives time to discuss complex cases in a multidisciplinary arena.
Our service offers secondary mental health care to adults living with dementia and people over the age of 65 who have been diagnosed with a mental illness. By providing recovery-focused care and treatment, we encourage individuals to adopt a strengths-based approach and offer support to achieve personal outcomes. Our team works closely with individuals and families to ensure they are safe and living well in their own communities.
