About the Role:
As an Intensive Support Worker, you will provide person-centred, trauma-informed, wellbeing-focused support in the community to promote independence and tenancy sustainability.
Main Responsibilities:
* Provide intervention and support for new referrals and existing supported individuals in supported and move-on accommodation.
* Deliver trauma-informed, person-centred, wellbeing-focused support.
* Monitor and evaluate support outcomes using our SHARP tool, ensuring compliance with legal and statutory standards.
* Assist individuals transitioning into the community, developing their tenancy and life skills.

* Passion for helping people is essential.
* Ideally, possess at least 1 year of experience working with vulnerable people and challenging behaviour.
* Commitment to care practice in line with SSSC, Care Standards, and organisational policies.

* Flexible and capable of lone working.
* Proficient in Microsoft applications.
* SVQ level 3 or above preferred.
