Case Manager
The Case Manager provides community-based services to correctional clients following their release, assisting with their transition back into the community. Responsibilities include monitoring clients in supervised apartments and transitional living programs. Clients are offenders under supervision by the Department of Corrections.
This is a part-time role requiring approximately 10-18 hours weekly, mostly during weekday mornings from 8 am to 11 am.
Responsibilities include, but are not limited to:
* Client intake and assessment
* Client evaluation
* Monitoring and reporting client progress
* Maintaining client files and notes
* Performing daily checks as per case plan
* Reporting violations or safety concerns promptly
* Teaching independent living skills
* Money management guidance
* Providing education and training support
* Referring clients to community resources
* Adhering to all rules and regulations
* Monitoring client whereabouts and security
* Reviewing security camera footage
* Completing activity records
* Purchasing supplies as needed
* Handling room turns and common area maintenance tasks
* Following supervisor’s instructions
Qualifications
Knowledge, Skills & Abilities:
* Owns a vehicle with valid driver’s license and proof of insurance
* Ability to travel frequently
* Sensitivity to diverse backgrounds and cultures
* Excellent communication skills
* Professional interaction with staff and clients
* Strong decision-making and self-motivation
* Maintains confidentiality
Education & Work Experience:
* High school diploma or equivalent
* Associate’s Degree in Criminal Justice, Human Services, Social Work, or related field preferred
* At least 1 year experience or education related to corrections or social services
* 3+ years experience with correctional clients, culturally competent
