An exciting opportunity to lead a team increasing independence and well-being for adults receiving social care support in Sheffield. The team consists of Prevention and Support workers that work with individuals to identify their aims and aspirations and provide individualised and focussed pieces of work to deliver on them.
Reporting to a service manager you will work closely with the first line manager to direct and monitor progress within the team, completing any social work actions that arise from the team's work and ensuring accurate and appropriate recording. You will receive referrals from future options social work teams, including Preparation for Adulthood, and you will use your professional judgement to prioritise and allocate these to the team.
