Package Description: We are seeking an experienced and motivated District Nurse Team Lead to join our community services, providing strong clinical and operational leadership within a busy and rewarding environment. You will work as part of the 24-hour district nursing service, under the direction of the district nursing Team Manager / Leader, providing high quality holistic care to patients. This is a full-time role working 37.5 hours per week based at The Hollies BA3 2DP.
Responsibilities
* Undertake responsibility for a delegated caseload under the supervision of the District Nursing Team Leader and, in the absence of the Team Leader, carry out initial assessment of care needs and the provision and evaluation of individual patient programmes of care including prescribing equipment and the discharge of patients from the caseload.
* Assume delegated responsibility for holistic assessment and the planning, implementation, monitoring and evaluation of programmes of care/care packages, contributing to complex assessment/care packages.
* Provide high quality palliative care to patients and their families, as required.
* Maintain clinical competency in a range of extended skills, as required to meet the needs of patients on the caseload.
Qualifications
* Registered General Nurse
* Diploma in health-related area or equivalent experience
* Experience working as part of a multi-disciplinary team or multi-agency
* Knowledge and experience of undertaking complex assessments and planning/evaluation of care packages
* Excellent written and verbal communication skills
* Evidence of effective interpersonal skills
* Ability to problem‑solve and make clinical decisions
* Knowledge of safeguarding adults and at‑risk vulnerable adults
* Excellent computer skills, including use of electronic clinical systems
* Non‑medical prescriber (desirable)
* Experience in clinical audit and Palliative Care (desirable)
* Understanding of clinical governance framework and implications for practice (desirable)
* Knowledge of lone working risks and associated risk management (desirable)
* Motivation to self‑develop
