Join the Home First Therapy Team in Buckinghamshire
An exciting opportunity has arisen to join the Home First Therapy team in Buckinghamshire. The team is still developing into a substantive service, offering team members the chance to influence and shape its growth.
We seek motivated and skilled individuals to support patients recently discharged from hospital, helping them regain independence and remain in their own homes.
Role Overview
The ideal candidate will adopt a holistic and person-centred approach to support patients' recovery from hospital admission and achieve their short-term goals, maximising independence. Responsibilities include:
1. Visiting patients at home to provide therapeutic treatments under the guidance of qualified therapists.
2. Assessing and delivering prompt therapeutic interventions as part of the Home First Therapy team.
3. Managing designated aspects of patient treatment protocols and treatments in patients' homes.
4. Serving as a technical resource regarding mobility equipment within the service.
5. Documenting, evaluating, and liaising with physiotherapists and occupational therapists post-intervention.
6. Completing and delegating non-clinical tasks within the team.
7. Working as part of the weekend Home First Service.
