Overview
The Macmillan Clinical Nurse Specialist will work as a member of the wider locality based multidisciplinary Manchester Macmillan Supportive and Palliative Care Service. The post holder will support Neighborhood teams to deliver locally driven high quality care, responsive to patient and carers needs. This includes education of other health care professionals, training, support and advice for the multidisciplinary team. The focus of the multidisciplinary approach will be to support patients and carers to live well, prevent inappropriate hospital admission and meet their preferred priorities for care.
An exciting opportunity has now arisen within the City Wide Manchester Macmillan Palliative Support Service for a Clinical Nurse Specialist in Palliative Care. The team are friendly and supportive; development opportunities are available. The post holder will work as part of the Macmillan Specialist multi-disciplinary team comprising Medics, Dietitian, Occupational Therapist, Physiotherapist, Nurses, Speech and Language Therapist, Assistant Practitioners, and admin support. The team aims to provide effective support for palliative care and/or rehabilitation to patients with a life-limiting condition requiring palliative intervention.
Responsibilities
* Provide supportive palliative care to enhance the quality of life for patients and carers with life-limiting illnesses requiring palliative care, including complex physical, psychological and emotional needs.
* Utilise extensive palliative medication knowledge to support complex symptom management needs of patients.
* Support Neighborhood/MDT teams with education, training, and advice for the multidisciplinary team.
* Collaborate as part of the Macmillan Specialist multidisciplinary team to deliver patient-centered care and rehabilitation as appropriate.
Qualifications / Requirements
Extensive palliative medication knowledge relevant to complex symptom management is required.
