Job Summary

We are looking for a Lead Nurse to join The Avenue Surgery team, to provide a specialist resource for patients within our practice in collaboration with other members of the multidisciplinary team.


Job Responsibilities

* Assisting in and performing routine tasks related to patient care as indicated by patient need
* Acting as a clinical leader in the delivery of practice nursing services to patients, ensuring that the needs of the patient are the priority
* Leading the nursing team in the planning and implementation of local guidelines, protocols and standards, and local projects or initiatives
* Promoting the role of the nursing team in the provision of care
* Participating in practice/clinical meetings, delivering the nursing agenda, and running nurse department meetings
* Supporting QoF strategy and delivery for long‑term condition management, maximising income
* Managing and maximising ICB, PHE and NHS England‑driven targets (childhood immunisations, flu campaigns, smears etc)
* Supporting the development of others to maximise staff potential
* Being the infection control lead, completing IPC audits, developing action plans and ensuring implementation is embedded
* Working closely with the ACP Clinical Lead
* Line‑managing the practice nurses, providing regular supervision and completing annual appraisals
* Recognising and working within own competence and professional code of conduct as regulated by the NMC
* Producing accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures
* Prioritising, organising and managing own workload in a manner that maintains and promotes quality
* Delivering care as an individual and team according to NSF, NICE guidelines and evidence‑based care
* Assessing effectiveness of care delivery through self‑ and peer review, benchmarking, and formal evaluation
* Initiating and participating in the maintenance of quality governance systems and processes across the organisation and its activities
* Utilising the audit cycle to evaluate the quality of work and implementing improvements where required
* Collaborating with other clinical teams to improve the quality of health care and developing the wider primary care multi‑disciplinary team
* Managing infection prevention and control (IPC) within local and national guidelines, including CQC requirements
* Communicating effectively with patients, carers and stakeholders, ensuring informed consent and providing support with difficult news
* Advocating for policies that reduce environmental health risks, are culturally sensitive and increase access to health care for all
* Using technology and appropriate software to support planning, implementation and monitoring of care, and to produce written documents for practice priorities
* Managing and assessing risk within the areas of responsibility, ensuring adequate measures are in place to protect staff and patients
* Providing educational support to patients, carers, families and colleagues in a learning environment
* Promoting equality, diversity and inclusion, and supporting patients from marginalised groups to access quality care
* Undertaking any required training in order to meet the needs of the practice


Qualifications

* Essential: Relevant 1st nursing degree / MSc, mentor/teaching qualification, evidence of continuous professional development
* Desirable: Independent Prescribing Course


Skills and Knowledge

* Awareness of accountability of own and others roles in a nurse‑led service
* Knowledge of health promotion strategies
* Knowledge of local and national health policy
* Knowledge of clinical governance issues in primary care
* Skills in learning from patient safety incidents and significant events
* Clinical leadership skills
* Change management skills and ability to support patients in behaviour change
* Skilled communicator, able to influence, negotiate and manage conflict, including difficult messages to patients and families
* Skills and knowledge in identifying and raising safeguarding concerns for adults and children
* Experience of delivering and implementing Infection Prevention and Control policies within Primary Care


Experience

* Essential: Working autonomously in a senior nursing role within Primary Care; experience of clinical audit; experience in nurse‑led management of long‑term conditions; experience in implementing protocols and clinical guidance; team leader/line‑management experience; teaching and mentoring experience in primary care
* Desirable: Experience in Quality Improvement Initiatives; community nursing experience


Person Specification

* A natural curiosity backed by excellent critical analytical skills
* Assertive and confident to challenge poor conduct and behaviours
* Strong patient focus
* Excellent time management, prioritisation and organisational skills; ability to manage and deliver to agreed deadlines
* Highly motivated and enthusiastic
