Main duties of the job

Your role and responsibilities

Suffolk County Council (SCC) delivers the 0 - 19 Healthy Child Service for the whole of Suffolk, which includes School Nursing, Health and Children's Centres, Family Nurse Partnership Services and our Health Business Centre.

Working with and supporting families in the crucial early years of a childs life, you will:

* Play a key role in ensuring the best possible start in life for children in Suffolk
* Have a profound impact on the lifelong health and wellbeing of young children and their families
* Help families and young children overcome the challenges that they face
* Provide expert advice, information, assessments, and interventions.

The team

We are a fully integrated service that can responsively support the needs of children and their families.SCC has invested in Staff Nurses to work alongside Health Visitors and Healthy Child Practitioners to facilitate improved public health outcomes.

We specialise in working with families with children aged 0 to 5 to identify health needs as early as possible to improve health and wellbeing. We promote health and by offering early interventions and advice, we support our families ever changing needs.

Job responsibilities

This post isdesigned to support the Health and Childrens Centre service by workingdirectly with families to help support the social, emotional, physical, andintellectual wellbeing of children from pregnancy to 5 years, through thedelivery of the Healthy Child Programme and Childrens Centre statutoryframework.

You will beexpected to plan and deliver effective and evidence based universal andtargeted services to children and families from pregnancy to 5 years, as wellas support the provision of an appropriate learning environment to enable otherpractitioners and students to develop, which will include mentoring.

This role willinclude group planning, delivery and evaluation as well as offering a homevisiting programme focusing on the needs of those families on a universal anduniversal plus pathway. This role be key in support parents to develop a strongbond with their child and remain positive role models as their children grow,understanding their role as their childs educator.

You will work inaccordance with the NMC Code of Professional Conduct, and other regulatorydocuments, ensuring your NMC registration is maintained.

You will directlysupport the needs of children and their families through the implementation ofthe care pathways, outlined in the Health & Childrens Centre StandardOperating Procedures (SOP) and in accordance with clinical governanceframework.

Through effectivepractice and multiagency working, you will effectively safeguard children andyoung people, which will include participation in safeguarding meetings andcontributing to assessment processes.

Typical Responsibilities

Workwith children, young people and families using Suffolk Signs of Safety SOS and Wellbeing framework

Applyan integrated, multi-agency approach to supporting and empowering children,young people and their families

Toensure a strong focus on prevention, early intervention and identification ofneed, using your experience, knowledge and skill to determine the level ofintervention required

Work inaccordance with regulatory and statuary documents, legal frameworks, NICEguidance and Suffolk County Council policy andprocedures

Activelybuild relationships and networks with other professional groups and services inthe locality to deliver a holistic service to children, young people and their families.

To workwith the community and ensure the active participating of children, youngpeople and families in service planning and delivery that respond to localneeds and reflects the diversity of the community and the inclusion ofvulnerable people and those with additional needs

Toidentify risk and safeguarding concernsand escalate where required, undertaking appropriate referrals, including CAF


Person Specification


Qualifications

* Current Nursing & Midwifery Council (NMC) Registered Nurse
* Evidence of continuing professional development
* Willingness to undertake further professional training and attend training courses as well as reading literature as required to update knowledge and skills
