South London and Maudsley NHS Foundation Trust is inviting applications for a Community Practitioner role within the Croydon Assessment & Liaison Team. The post is open to registered Mental Health Nurses, Social Workers, and Occupational Therapists. The role involves delivering short‑term community mental health assessments and care for individuals aged 18‑65 in Croydon.
Responsibilities
* Provide assessment and planning of care to a group of service users within the team.
* Maintain accurate records associated with assessments and care plans.
* Work effectively within a multidisciplinary team and across other agencies.
* Deliver high‑quality care to individuals with complex biopsychosocial needs.
* Adhere to professional standards and codes of conduct.
* Provide supervision to junior staff and students.
* Support the team leader and act as team leader in their absence.
Qualifications
* Registered Nurse (Mental Health) or equivalent professional registration (Social Work or Occupational Therapy).
* Completion of a preceptorship course.
Experience
* Experience working in community mental health settings (e.g., Home Treatment Team, CMHT) or acute inpatient settings.
* Experience in managing safeguarding issues.
* Experience in carrying out detailed assessments, formulating and planning care.
* Experience working in a multidisciplinary team setting.
* Experience managing a caseload.
Knowledge
* Knowledge of evidence‑based interventions for people with complex mental health problems.
* Awareness of bio‑psycho‑social assessments and safe and effective risk management.
* Up‑to‑date knowledge of relevant legislation.
* Knowledge of local organisations to support recovery plans for service users.
Attributes
* Recognition of own limitations, strengths, and weaknesses with a willingness to seek advice.
* Methodical approach to work with attention to detail.
* Ability to use computerised information and communication systems.
