We are delighted to invite applications for a Deputy Team Manager role within our newly established Assertive Outreach Team (AOT). This innovative service will provide high‑quality, person‑centred care to individuals with complex mental health needs who may struggle to engage with traditional services. The role is based in Oxfordshire and involves working flexibly across the City of Oxford, supporting individuals in community settings, their homes or Trust premises.
Responsibilities
* Act as a clinical expert, advancing the development of clinical practice.
* Provide evidence‑based holistic patient care within a variety of settings, from assessment through to diagnosis, treatment and discharge, including medication administration within agreed guidelines.
* Complete all relevant PROMS in the planning and implementation of care plans.
* Diagnose, assess, plan and implement patient‑specific programmes of care/interventions, contributing to continuous assessment.
* Perform advanced clinical skills, including taking a clinical history and evaluating information to produce an appropriate differential diagnosis.
* Act as a resource for the non‑medical team within the community directorate, the Trust and the wider health community as required.
