We are looking to recruit a Qualified Adults Social Worker in the Hospital Discharge Team within Hackney. The successful candidate will be working in a well-structured, organised team who are growing their social worker staffing levels.This is a fantastic opportunity to join a stable working environment with experienced leadership, whilst receiving regular supervision and training opportunities. * Assess the social care needs of adults in an acute hospital setting to support timely, safe discharge
* Complete Care Act assessments, mental capacity assessments, and best interest decisions in line with legislation
* Work collaboratively with health professionals, families, and partner agencies to coordinate discharge planning
* Arrange appropriate packages of care, placements, and community support services
* Safeguard adults at risk and respond to concerns in line with statutory duties and local procedures
* Contribute to multidisciplinary team meetings and discharge planning forums
* Maintain accurate, timely, and legally compliant case records
* Promote strengths-based, person-centred practice and support service user choice and independence
* Manage a complex caseload, prioritising effectively to meet service demands and discharge targets.
The ideal candidate will have:
* Social Work England registration
* Eligibility to work in the UK
* Social Work qualified (MA/BSc/DipSW/CQSW/MSc/BA)
