Posted: 10 May
Offer description

An exciting opportunity has arisen for a Band 5 Care Practitioner to join the Out of Hours District Nursing Team. The service operates across Sefton Place from 8pm to 8am, 7 days a week. Working in line with Mersey Care values, you will demonstrate excellent empathy, communication skills, and a person-centred approach. You should be eager to continue learning and developing the competencies necessary to provide evidence-based care to patients in their own homes. Clinical decision-making skills and a problem-solving approach are essential for this role. You will be a good team player, capable of working independently, escalating issues appropriately, and reporting back as needed.

You will also have the opportunity to be mentored, with full support provided to develop and enhance your skills.

Role

As a result of the post-holder's effectiveness, the Trust expects to see outcomes such as:

* Mersey Care NHS Foundation Trust as a leading provider of community services, mental health care, physical health, addiction services, and learning disability care.
* Service users receiving high-quality, stigma-free, discrimination-free, and harm-free services.
* Staff engaged in the delivery, innovation, and continuous improvement of services for the benefit of service users.
* Visible and responsive leadership, setting standards and role-modelling best practices across the division.
* The Trust's core values of Continuous Improvement, Accountability, Respectfulness, Enthusiasm, and Support embedded across the division and evident to service users.

Mersey Care is one of the largest trusts providing physical and mental health services in the North West, serving over 1.4 million people, with additional commissioned services covering North West, North Wales, and the Midlands.

We offer specialist inpatient and community services supporting physical and mental health, including high secure mental health facilities. Our commitment is to ‘perfect care’ — safe, effective, positively experienced, timely, equitable, and efficient. We support staff to excel and work collaboratively with service users, families, and carers to shape future services. We are currently implementing a programme of organisational and service transformation to improve quality and reduce costs.

Flexible working requests will be considered for all roles.
