Responsibilities
* Conducting detailed assessments to develop safe, personalised care plans.
* Collaborating with Occupational Therapists, support managers, and mental health teams.
* Facilitating placements into our new reablement and step-down facility.
* Reviewing and adjusting care plans to adapt to changing needs.
* Maintaining accurate records in line with local policy and best practices.

Qualifications
* A Social Work Qualification.
* Registration with Social Work England (SWE).
* Experience in coordinating with healthcare and social care professionals.
* Strong assessment and care planning skills.
