Senior Physiotherapist to join the Intermediate Care Team in the South, based in Cambridgeshire.

The vision of the service is to provide outstanding, patient focused, community based health care and therapy, optimising peoples potential to achieve their goals by supporting earlier discharge from hospital using the home first model.
Responsibilities
* Provide comprehensive, holistic therapy assessment and rehabilitation to patients within their home environment, maximising independence within a 4-week pathway.
* Hold clinical responsibility for patients within the service and Therapy Practitioners within the team; prioritise workload including supervision of Therapy Assistants within the team.
* Work proactively, in collaboration with the wider multidisciplinary team to ensure patient flow and safety.
* Assist in the development of Integrated Care Workers and Therapy Practitioners to support the clinical delivery of the service.
