Young People Leaving Care Outreach Support Worker
This is a long-term temporary position with no fixed end date, suitable for quick start through a temp-to-permanent placement if you are subscribed to the update service and have a short notice period.

Requirements and Responsibilities
1. Experience supporting young people and their families.
2. Full UK Driving License with Business Insurance is essential.
3. Flexibility to work hours between 8am and 8pm, Monday to Friday, to meet the needs of the young people.
4. Completing property inspections.
5. Supporting young people to develop independence skills.
6. Managing diaries and working closely with permanent staff.
7. Experience in similar settings and preferably NVQ3 Children & Young People Workforce or equivalent qualification.
8. High standard of general education; preferable NVQ Level 3 Health & Social Care – Children and Young People (CCYP) or equivalent.
9. Willingness to undertake further training.
Job Purpose
To provide support for vulnerable young people in line with care plans, assisting in their development and well-being.
Main Duties & Responsibilities
1. Assist the Young person’s Social Worker in service delivery, including:
* Providing transport and escort services as directed.
* Conducting welcome and induction procedures.
* Assessing Service Users’ needs.
* Supporting and monitoring Service Users.
* Assisting with welfare and NHS issues.
* Preparing reports and arranging appointments with partner agencies.
* Maintaining contact with Service Users and informing them of support groups.
* Supporting Move-On processes.
1. Maintain accurate electronic records of Service Users.
2. Attend meetings and participate in team activities.
3. Provide mediation and support as required.
4. Use IT packages effectively.
5. Work cooperatively across multidisciplinary teams.
6. Maintain confidentiality at all times.
7. Assist in policy and procedure development.
8. Report housing issues and assist in defect resolution.
9. Undertake On-Call duties as required.
10. Participate in office reception duties if needed.
