Senior Clinical Lead Nurse – South Regions
Libertatem offers complex care services for children and adults who have conditions including brain injuries, spinal injuries, cerebral palsy, learning disabilities, autism, and mental health needs. We are looking for an experienced nurse to take on the role of Senior Clinical Lead, managing a small team of Clinical Leads across our South region. You will oversee client packages in the area based on clients’ needs and plan your time accordingly.
Principal Accountabilities
* Deliver personalised client-centred service outcomes, carry out care assessments, write care plans, and manage competencies.
* Provide leadership to the Clinical Leads and support staff, working with Regional Operations Managers to deliver excellent care delivery.
* Report to the Head of Clinical Services and Registered Manager.
* Collaborate with Field Care Managers, directing aspects of their work where needed.
* Maintain professional skills and undertake required training and development for personal growth.
* Work in accordance with national standards, legislative requirements, relevant regulations, and best practices.
You’ll Have The Opportunity To
* Oversee and manage the Clinical Leads to maintain standards and compliance.
* Monitor client and family-centred outcomes and implement changes proactively.
* Support the Head of Clinical Services and Registered Manager to strengthen clinical governance and community services.
* Provide escalation support to the clinical team to deliver high-quality care within care packages.
* Develop individualised care plans with clients, families, and other healthcare professionals to ensure goals are patient-focused and achievable.
* Undertake initial holistic nursing assessments with clients, families, and carers, involving other providers as needed.
* Support the region by visiting clients, especially where complexity arises, and observe practice of Clinical Leads during visits.
* Engage with Regional Operations Managers regularly to ensure a holistic, client-centred approach.
* Ensure all recording systems and documentation are up to date and of high quality.
* Motivate and manage your nursing/care team, aligning with the company’s values and fostering pride in work.
* Support field-based care workers and nurses through regular supervision, competency assessments, and appraisals.
* Report against targets and standards and implement action plans for service improvements that impact clients and service quality.
* Commit to quality standards to ensure compliance with statutory, mandatory, and contractual requirements.
Working With Others
* Collaborate with Central Support (including HR) to motivate staff through supervision and appraisals.
* Liaise with Care Coordination on rotas and ensure staff are trained, assessed, and competent for lone working.
* Promote and protect client welfare.
* Work with multidisciplinary teams to maximise client outcomes.
* Identify improvements and share ideas with Libertatem’s senior management.
Other Duties
* Seek opportunities for personal and professional growth and development.
* Adhere to the NMC Code of Conduct in all professional duties and relationships.
* Stay up to date with best practices and evidence-based research.
* Attend statutory training and any other trainings as directed.
* Be registered with the NMC and support revalidation in a timely manner.
* Understand the regulatory framework governing Libertatem’s services.
* Be part of the escalation on-call rota outside office hours.
A Bit About You
This role blends clinical nursing skills with coaching and people management. Experience delivering complex community care is essential. Experience with spinal injuries, acquired brain injuries, cerebral palsy, learning disabilities, autism, and mental health is desirable. You should be able to pass on knowledge to others and work with both adults and paediatric care; training will be provided where required.
* Professional nursing registration with no restrictions
* Relevant clinical experience – ABI/Spinal Injury/Complex Care/Community Nursing
* Ability to develop effective working relationships with key stakeholders such as case managers, Integrated Care Systems, General Practitioners, consultants, and other healthcare professionals
* Experience of people management and leading teams
* Experience in clinical governance and quality improvement activities
* Experience in incident and complaint management
* Passion for delivering outstanding care in a challenging but rewarding environment
* Excellent communication skills, clear and confident, with high standards of written documentation
* Positive, solution-focused approach
* Strong commitment to quality standards and statutory, mandatory, and contractual requirements
* Flexibility and pragmatism; ability to self-plan and adapt to shifting priorities
* Remain calm and professional during busy periods; able to work effectively in a team
* Proficient IT skills
* Driving licence
