We need a permanent, full-time x4 Social Worker's to work across West Lancashire (Based in Ormskirk) as part of our Adult Services Older Adults and Physical Disability Community Teams.

We’re driving forward innovative practice that will make a difference to our communities, and we need new Social Workers to help us achieve our vision of supporting people to live independently in a place they call their home.
As part of your new role, you will be expected to undertake strength-based, outcome-focused assessments and support planning, ensuring that the residents of Lancashire receive the right level of support.
You will play a vital role in supporting people to access local services, maintain independence, promote health and wellbeing, and live independently. A key part of the role includes working alongside other agencies, professionals, and informal carers to provide a holistic service.
In return for your commitment and hard work, you will work alongside supportive managers and be offered various training and progression opportunities.
