Physiotherapy Band 5 Position at Pulse
The Physiotherapy team at Pulse is currently recruiting for a Band 5 Physiotherapist in Watford. Previous experience of working in NHS is essential for this role.
Position: Physiotherapist
Banding: 6 and 7
Payrate: £24-27 per hour
Location: Watford
Hours: 7 days a week from 08:00 to 20:00, including bank holidays. You will be expected to work on rota’d weekends, bank holidays, and late shifts as required by the service.
Duration: Ongoing
Essential Skills:
* HCPC registered
* Lead discharge assessment and planning for patients on the wards, from advanced planning to onward transfer, using a proactive case management approach based on professional background (nursing, occupational therapy, physiotherapy, or discharge skills)
* Collaborate with the Transfer of Care Hub team, ward-based multidisciplinary teams, and community health and social care partners to agree on complex discharge plans that reflect ongoing care needs and patient wishes
* Lead on coordinating early discharge to ensure patients leave hospital within 24 hours, are medically optimized, and ready for discharge to minimize delays, improve inpatient flow, and reduce clinical deconditioning
* Identify and escalate potential discharge delays, leading problem-solving and case review meetings as needed
* Promote safe and effective discharge practices throughout the Trust and serve as a resource for expert advice and guidance
* Relevant qualification in Physiotherapy
