This is a rare and exciting opportunity for you to come and join our friendly team.

We are a multi agency team which provide health and wellbeing education and promotion to adults and older people in their own homes, over the telephone and via other routes i.e. group sessions and community events.

We also help deliver the NHS Health Checks program in the community and at outreach venues across West Sussex.

This is a great role for a community nurse with the enthusiasm for Health Promotion. It's a job that comes with flexibility and the scope for you to bring in your ideas on how to reduce health inequalities across West Sussex.


Main duties of the job

NHS Nurse Advisor for Adults

Fixed term for 9 months.

The Nurse Advisor role is varied, focusing on home/telephone assessments, NHS Health Checks, and outreach. The Advisor completes holistic assessments of patients' needs either by phone or during home visits. After appropriate training, they deliver NHS Health Checks in community settings, supporting individuals in accessing health and wellbeing services through behaviour change--part of services commissioned by West Sussex County Council's Public Health Department.

The role involves close collaboration with PAT colleagues, primary care teams, social services, and the voluntary sector to ensure integrated service delivery and avoid duplication. Nurse Advisors also help cover the Duty Line, support the maintenance of a service directory for accurate signposting, and ensure resources target populations with the greatest need--guided by Public Health data and the multi-agency PAT Board.

While not a managerial role, the post holder may occasionally supervise student nurses during community placements.


Job responsibilities

* Provide a holistic assessment of health, social and environmental needs of adults and older people living in the community.
* Provide advice, signposting and onward referral to a range of statutory, voluntary and community services to enhance the health and wellbeing of adults and older people by utilising up to date knowledge of relevant national & local policy developments.
* Work as part of a multi-agency team which includes health, social and caring services and voluntary sector staff.
* Attend regular PAT & NHS Nurse Advisor team meetings, sharing lessons & supporting the development of new services.
* In conjunction with other multi-agency colleagues continue to develop, expand and implement a health enhancing, population based approach to improving the health and wellbeing of adults and older people in the community.
* Using a robust evidence based approach, working with the Wellbeing Hubs, actively plan and implement activities designed to promote independence and wellbeing amongst adults and older people living in the community.
* Keep up to date and contemporaneous records adhering to National and Trust policies and procedures.
* Demonstrate commitment to continual professional development, participating in clinical supervision and working within the NMC Code and Sussex Community NHS Foundation Trust policies.
* Attend all statutory and mandatory training and any other training required for the role.


Person Specification


Qualifications

* Registered nurse with the NMC


Experience

* Relevant experience of working with adults and older people in community settings.
* Evidence of post qualification experience and/or further education in health/social care related subjects.


Skills and Knowledge

* Ability to demonstrate knowledge/understanding of the boundaries of the role.
* Ability to demonstrate an understanding of clinical governance.
* Ability to work as an effective member of a team but with ability to work independently & use initiative.
* Understanding of statutory Health & Social Care Services. i.e The Care Act 2014. National Service Frameworks for Older People and related documents.
* Knowledge of the NHS Health Checks Programme.
* Clear understanding of multidisciplinary working in the care of adults & older people.
* Ability to provide & receive complex, sensitive information.
* Good knowledge of computing skills e.g. Windows.
* Ability to deal with a wide range of people at all levels, including clients, carers, health & social care colleagues & other professionals.
* Ability to demonstrate knowledge of NMC guidelines.


Other Role Requirements

* Empathetic & non-judgemental approach.
* Car driver with access to a car and a current driving licence, unless under Equality Act.
* Flexible re: time management.
* Willing to learn new skills.

£32,073 to £39,043 a year per annum / pro rata
