Registered Mental Health Nurse / Occupational Therapist
Main Responsibilities:
1. Foster recovery through empowerment of service users and carers by using a strengths-based approach involving the service user and carer in the planning of their care.
2. Establish and maintain consistent supportive relationships, encouraging increased self-determination and responsibility for service users and their care.
3. Offer a range of evidence-based interventions and risk management approaches to help service users and carers reduce and manage symptoms and high-risk behaviour during episodes of relapse.
4. Be proactive, innovative, and creative in delivering care, facilitating service users' recovery.
5. Engage service users and carers in relapse prevention and risk management/contingency plans, providing interventions that maximize the service user's ability to resolve relapse crises and prevent hospital admission.
6. Support discharge planning by liaising with inpatient units and facilitating early discharge from inpatients and the CRHT Team through regular reviews.
7. Deliver a flexible, responsive service based on service users'/carers' needs.
8. Collaborate effectively with the multidisciplinary team and other agencies.
9. Provide high-quality care, solutions, and signposting for service users with severe and enduring mental health problems.
10. Provide care within the Strength-Based Model, maintaining service users within their community environment.
11. Act as an educator for service users and carers, advising on mental health issues to promote a positive health approach and enhance self-management of illness, symptoms, and relapse.
12. Respond swiftly to urgent referrals with interventions aimed at preventing admission or re-admission.
13. Work closely with carers, recognizing racial, cultural, and sexual preferences, and addressing health inequalities.
14. Ensure accurate record keeping and documentation reflecting service users' needs.
15. Maximize resource utilization within the team and community.
16. Assess, plan, implement, and evaluate care using evidence-based tools and models.
17. Work within the MDT framework according to the code of practice to ensure optimal care.
18. Review and update care plans regularly to incorporate the latest information and service user and carer views.
19. Undertake risk assessments and manage risks for caseload service users, sharing information appropriately.
20. Ensure timely activity/contact entries on RiO, conforming to data quality requirements.
21. Develop care plans aligned with national guidelines.
Leadership:
1. Participate in developing an effective team and fostering productive relationships organization-wide.
2. Promote integrated health professional working internally and externally.
3. Support a positive team culture by managing conflicts effectively.
4. Engage in organizational working groups to improve service delivery, protocols, and guidelines.
5. Participate in audits and clinical governance activities.
6. Share knowledge through literature and peer reviews, encouraging practice development.
7. Address unprofessional practice and mentor new staff members.
8. Review staff performance and serve as a positive role model.
Clinical Skills:
1. Maintain knowledge of current legislation.

4. Administer medicines as per policies and procedures.

6. Carry out physical health checks and follow up as necessary.

1. Communicate effectively with MDT members regarding assessments, care plans, and risk factors.
2. Handle sensitive information professionally and communicate appropriately.
3. Respond to emergencies with sensitivity and clarity.
Training:
1. Act as a mentor to students, supporting their development and high standards of care.
2. Support ongoing professional development and lifelong learning.
3. Plan and undertake mandatory training and workshops.
4. Participate in appraisals and develop personal development plans.
5. Support new staff integration and training initiatives.
