Are you an experienced, enthusiastic Advanced Clinical Practitioner (ACP) looking for a new career opportunity in Primary Care? Join our dynamic GP practices in Morpeth as part of an experienced, proactive, and friendly team comprising GPs, Practice Nurses, Nursing Associates, and Admin Staff.
As an ACP, you will play a vital role in the assessment and delivery of excellent care to patients with acute and chronic conditions, providing autonomous care, triage, telephone support, and home visits.
We are looking for a highly motivated, enthusiastic team player who can demonstrate high standards of clinical excellence and work across multiple sites covering extended access provision.

Responsibilities
* Assess and deliver patient care for acute and chronic conditions.
* Triage patients effectively, provide telephone advice and support, and welcome patients for appointments.
* Support care provided to local Care Home and complete home visits when required.
* Provide leadership, support, and mentorship to junior team members; supervise and assist with their development.
* Collaborate with the Practice Lead to appraise junior nursing team members.
* Manage patients over a period of time, organise appropriate services, and deploy resources effectively.
* Contribute to the Health Centre’s work by delivering high‑quality evidence‑based care and autonomous clinical decision‑making.
* Work across sites and cover extended access provision.
Qualifications and Requirements
* Valid driving licence with ability to use a car for work purposes or Trust personal lease vehicle.
* Experienced, autonomous ACP with significant, relevant clinical experience.
* Strong clinical reasoning, diagnostic skills, and ability to manage patients independently.
* Exemplary communicative, team‑working, and leadership skills.
