Hours:

We are looking for a dedicated and motivated Care Assessor to join our team. This role is ideal for someone who is passionate about helping people maintain their independence, health, and wellbeing within the community.

This position is a split role, with approximately 50% of your time spent in the field conducting assessments and visits, and 50% office-based supporting care operations.

About the Role:

Reporting to the Care Manager, the Care Assessor is responsible for managing new care packages, ensuring person-centred assessments, and maintaining compliance standards. You will play a vital role in client care by ensuring every individual receives a service tailored to their needs.

Key responsibilities include:

8. Carrying out Individual Needs Assessments for new and potential clients
9. Completing and updating risk assessments and person-centred care plans
10. Conducting introductory visits for new care packages
11. Ensuring all client visits are covered at all times
12. Supporting the Field Care Supervisor (FCS) and providing cover when needed
13. Assisting with compliance and audits alongside the Care Manager
14. Conducting routine reviews of client care to ensure quality standards
15. Monitoring client and Care & Support Worker satisfaction
16. Ensuring documentation (home and electronic) is accurate and complete
17. Supporting training activities when required

Requirements:

18. Full driving licence and access to own vehicle
19. Degree-level qualification
20. Experience in care or a related field (Care Coordinator experience preferred)
21. Strong communication and organisational skills
22. Ability to manage both field and office-based responsibilities
23. Passion for delivering person-centred care
24. Training will be provided
