An exciting and rare opportunity has arisen in South East Essex for an experienced band 6 Occupational Therapist to join our vibrant multi‑professional Urgent Community Response Team (UCRT) for a 7‑month secondment. This post offers excellent opportunities for on‑going professional development.
Responsibilities
The post holder will work within the UCRT as part of the wider Integrated Care Team to assist in the prevention of avoidable hospital admissions of patients in crisis or in need of rehabilitation and reablement. The role requires rapid response functional assessment and intervention to support patients to remain safe and independent at home. The therapist will work closely with nursing colleagues and other multidisciplinary team members, provide holistic functional assessments, make clinical judgements under the guidance of the Clinical Lead OT and team leaders, and deliver care with dignity, respect and compassion.
