Overview
We are looking to recruit an Advanced Clinical/Nurse Practitioner for up to 30 hours per week at Aelfgar Surgery, Rugeley, Staffordshire.
We serve a patient population of around 7,000 in a suburban area with semi-rural locations. The post-holder will manage a caseload in a primary care setting, ensuring high standards of care for all registered and temporary patients.
Responsibilities
* Provide care for presenting patients from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care within professional boundaries.
* Demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills for patients within general practice.
* Work collaboratively with the Practice team to meet patient needs; triage and manage patient requests, including same-day triage and referrals to the primary care team as appropriate.
* Diagnose and manage both acute and chronic conditions, integrating drug and non-drug treatments into management plans.
* Prescribe and review medications in line with policy, patient group directives, NICE guidelines, local care pathways and practice protocols.
* Support patient adherence to prescribed treatments and provide information on medications, side effects and interactions.
* Prioritise health problems and intervene in complex, urgent, or emergency situations, initiating appropriate emergency care when needed.
* Promote health by encouraging healthy lifestyles and self-care; assess and refer patients presenting with mental health needs; participate in vaccination and immunisation programmes.
* Provide opportunistic/continuing wound care and undertake home visits as appropriate.
* Ensure practice activities are safe, effective and within the practitioner’s boundaries of competence.
Qualifications and Experience
Essential
* Professional registration with NMC or HCPC
* MSc in Advanced Clinical Practice (or equivalent)
* Independent Prescribing or postgraduate study in pharmacology & therapeutics
* Management of minor illness, minor ailments and injuries
* Evidence of CPD in autonomous practice and ability to evaluate safety and effectiveness of own practice
* Chronic disease management; experience in telephone triage and home visits
Desirable
* Previous primary care experience
* Interpreting and implementing local/national policy; supervisory/management experience
* Experience in clinical audit, evaluation research and implementation of evidence-based practice
* Experience using medical software; networking and influencing skills; strategic thinking
* Awareness of clinical governance issues in primary care; self-direction and self-motivation
