We are offering an exciting and fast paced role at West Leeds Primary Care Network (WLPCN) for a prescribing nurse or nurse practitioner to join our highly skilled and experienced home visiting team. This is for the Acute Visiting element of the service; we undertake to see as many of the acutely unwell patients across our member practices as we can in order to alleviate pressure on the GPs within our practices. It is a highly valued service and as such we need a really robust and experienced NMC registered clinician to undertake this work. Job responsibilities
This role will be mainly reactive work forwhich we need an experienced Independent Nurse Prescriber with a good breadthof general healthcare experience who can undertake acute case home visiting. You will be part of a very experienced teamand there will be plenty of debrief opportunities with other clinicians,including prescribing Advanced Clinical Practitioners within your own team. You will be assigned a clinical supervisor tohelp with your continued professional development and training opportunitieswill be available from time to time. Yourresponsibilities will include the assessment and treatment of acutely unwell patientswho will be booked on to your ledger after having contacted their own GP surgery.You will have a fixed number of home visits per day once you have completedyour induction period.

Youwill have the opportunity to work collaboratively with other specialists /clinical disciplines within our organisation in a bid to provide truly holisticcare, improving patient outcomes.
Youwill be responsible for assessing acute conditions in the patients residence, providingtreatment and safety netting as well as actioning appropriate onward referrals(such as the Virtual Home Ward).
Youwill have access to a GP on call for immediate advice and guidance should youneed it.
Youwill manage your own visits based on prioritising the needs of the patient. Youmay be responsible for organising hospital admission should it be needed.
Youwill be expected to work independently in the patients residence and takeresponsibility for your own clinical decisions.
Youwill have access to multiple clinical specialists who can advise on a care planfollowing your clinical assessment.
Youwill be exposed to a variety of acute conditions such as UTIs, chest infections,oedema, infected wounds, migraines etc. as well as exacerbation of chronicillness. Experience in these areas will be essential
Youwill maintain accurate clinical records of all patient consultations andrelated work
The provision of this service is vital to ourmember practices and frees up practice GP time while we fulfil these visits forthem

* GCSE grade A to C in English and Maths
* NMC registered / holding a Nursing Diploma or Degree
