Health and Social Care Coordinator

Main duties of the job

Key responsibilities:
* Act as the primary point of contact for patients, carers and professionals, supporting complex health and social care needs.
* Support house‑bound patients with long‑term conditions to improve wellbeing and maintain independence.
* Respond to referrals, offer advice, support and signpost to other services to reduce unnecessary hospital admissions.
* Manage your own caseload, plan outcomes and expectations with patients and carers.
* Build strong relationships across multiple services and collaborate with Acute Trusts, Primary Care, Social Services and voluntary organisations.
* Communicate effectively, organise efficiently and analyse data to inform care plans.
Additional working pattern: 22.5 hours / 3x7.5 hours Monday to Friday.

Qualifications, training and experience
* Level 4 qualification (e.g. Certificate of Higher Education, Higher National Certificate, NVQ level 4) or equivalent experience.
* Functional Skills at Level 2 in Literacy and Numeracy (GCSE grade A‑C or equivalent).
* Recent experience of team‑working and versatility in a variety of tasks.
* A full, valid UK driving licence or a driving licence issued by an EU country (not exchanged from a non‑EU country).
Knowledge and skills
* Excellent communication skills, both verbal and written.
* Customer‑focused approach and a commitment to providing the best possible experience for patients and families.
* Proficiency in IT systems and databases.
* Awareness of developments within the NHS.
