Job Summary
The Care Navigator plays a pivotal role in guiding patients through their healthcare journey, ensuring they receive the appropriate care and resources tailored to their individual needs. This position requires a compassionate individual with a strong background in clinical settings, who can effectively communicate with patients, families, and healthcare providers. The Care Navigator will facilitate access to services, provide education on health conditions, and support patients in making informed decisions about their care.
Duties
* Assess patient needs and develop personalised care plans that align with their health goals.
* Coordinate with healthcare professionals to ensure seamless transitions of care across various settings, including hospitals, nursing homes, and home care.
* Provide education to patients and families regarding medical conditions, treatment options, and available resources.
* Assist in navigating public health services and community resources that support patient well-being.
* Maintain accurate documentation of patient interactions and care plans within the electronic health record system.
* Collaborate with multidisciplinary teams to enhance patient outcomes and improve service delivery.
* Stay informed about advancements in clinical research, medical imaging, and public health trends that may impact patient care.
Qualifications
* Previous experience in a clinical healthcare setting.
* Familiarity with public health initiatives and community resources is beneficial.
* Excellent communication skills, both verbal and written, with the ability to engage effectively with diverse populations.
* Proven ability to work collaboratively within a team environment while also being self-motivated.
* Have experience using IT and knowledge of using Windows package.
* A minimum of 5 GCSE'S to include English & Maths

Pay: £20,000.00-£20,209.50 per year
