We have an exciting opportunity within our emerging Intermediate Care Team for a skilled and dedicated Band 6 Occupational Therapist to work in the Dartford, Gravesham, Swanley, and Swale areas of Kent. The role involves providing specialist assessment, diagnosis, and therapeutic interventions for adult patients living in the community with complex and complicated care needs, referred into the service.
Additionally, you'll provide advice, support, and education for patients and their families/carers, voluntary agencies, and all those working with the patient regarding their medical condition and how this affects their lifestyle and environment. The goal is to maintain relationships, roles, and independence.
This is a part-time position working 34 hours per week, with some flexibility on how the hours are distributed across Monday to Friday. A full UK driving license and access to a vehicle for work purposes are essential for this position.
Main duties of the job
As a Band 6 Occupational Therapist, you will:
* Maintain high standards within the department by contributing to individual and service objectives.
* Provide clinical support and supervision to junior colleagues and offer training to students.
* Work independently and as part of a team, participating in innovation and service development within the pulmonary rehab service to benefit patients and colleagues.
* Complete complex holistic assessments in managing patients with chronic respiratory conditions.
