0.5 FTE of the role will support Travel with Confidence project;Delivering advice and form filling support.To support with organising and preparation of engagement, information, one to one andtraining sessionsTo assist therapists in delivering group and one to one sessions.To collate information relevant to travel and share with team.To support with booking of clients for one to one and group sessions.
In addition you will support our OT and SLT team with face to face activities and administration.

To support our occupational therapist and speech and language therapist with individual and group sessions plus research, information provision, administration and appointment booking.

JOB PURPOSE:To work with the INS team to support service users to maintain independence, reduce socialisolation and improving well-being for clients with long-term conditions and their carers. Thisis achieved through carrying out a range of activities and supporting the therapy team withgroup and one to one, face to face and online work.

DUTIES AND RESPONSIBILITIESClinicalTo assist with the setup, planning and execution of INS weekly group programme asinstructed by therapy team.Under supervision of Occupational Therapist (OT), Physiotherapist (PT) or Speech andLanguage Therapist (SALT), evaluate client's progress and report back to supervisingpractitioner when appropriate.To undertake administrative tasks to support clinical teamUndertake telephone/face to face information gathering activities.To support off site activities and satellitesTo support external group providers. To support trained volunteers within group setting.To lead group sessions for INS clients.To complete specific assessments with clients following instruction/teaching/induction bytherapists; for example, Well-Being Scale, MOCA, physical outcome measuresTo provide administrative support to the therapy/counselling service as requiredTo support office manager with reception/dealing with client and carer queries.To research, collate and distribute useful information to staff and service users.To undertake feedback projects.CommunicationTo communicate, in a professional and sensitive manner, with service users, volunteersand other staff in a way that respects their views, autonomy and culture.To provide explanations about the intervention carried out and overcome barriers toparticipation/acceptance where possible.To use knowledge and experience to deal appropriately with client's questions andconcerns, using judgement on the need to involve supervising practitioner.To attend and participate in team meetings and training as required.To report to and discuss with the Occupational Therapist, Physiotherapist and other teammembers, the clients' capabilities, performance, progress, and responses to rehabilitationprogrammes.To communicate effectively with clients and relatives/carers to gather information about socialand environmental circumstances and functional abilities and provide information aboutintervention and other relevant Services.To record all client interventions in the client's record in a timely way, in line with HCPCstandards of documentation.To work effectively as part of a multi disciplinary integrated team.To liaise with other services and agencies as required, e.g., social services and communityrehabilitation teams.Supervision and appraisalTo meet regularly with a supervisor to review and reflect on your own practice andperformance through regular participation in supervision and appraisal.Training staff, students, and volunteersTo assist with the induction, training and education of students and other staff in the worksetting.To participate in relevant training and development opportunities.
