Main Responsibility
* As a Care Coordinator, you’ll join a team focused on delivering impeccable customer service to our staff and service users, making a real difference.
* The Care Co-Ordination Centre operates 7 days a week from 8am to 8pm. The role requires working various sessions during these hours, including weekends, Bank Holidays, Christmas, New Year, and Easter Holidays.
* We offer a full-time opportunity to join our team.
* The post holder will be the first point of contact within HCRG Care Group for receiving and passing on referrals within expected timeframes from service users, GPs, and the Integrated Health and Social Care teams across North Kent.
The Ideal Candidate
You will have:
* Excellent interpersonal and communication skills, both verbal and written, with the ability to engage with a range of people daily
* Administrative experience in a busy, customer-facing environment
* A flexible approach to work
* An excellent telephone manner
* The ability to work effectively as part of a team
* Good organisational skills
* Confidence in using IT systems with working knowledge of Microsoft Office packages such as Outlook, Word, and Excel
