Overview
This is a full-time hybrid role for a Case Manager, based in Tewkesbury with flexibility for some remote work. You will coordinate and manage individual rehabilitation cases, assess client needs, develop and execute rehabilitation plans, liaise with healthcare providers, conduct regular follow-ups, and ensure clients are progressing toward recovery goals. You will document case activities and outcomes, ensuring compliance with regulatory and company standards.

Qualifications

* Experience in case management, rehabilitation, or clinical roles
* Ability to work both independently and collaboratively in a hybrid work environment
* Knowledge of health and wellbeing initiatives, with a passion for improving client outcomes
* Strong organizational skills and attention to detail
* Relevant degree or certification in healthcare, case management, or a related field
* Experience with data insights and metrics to track and report on client progress is a plus
