We are seeking a dedicated and compassionate Care Coordinator to join our care at home team. Responsibilities
* Act as the primary point of contact for service users and their families, providing guidance and support throughout their care journey.
* Coordinate care worker schedules, and follow-up care with various healthcare professionals.
* Maintain accurate and up-to-date service users records in accordance with data protection regulations.
* Liaise with medical staff to ensure service user needs are met efficiently and effectively.
* Assist in organising support services tailored to individual service user requirements.
* Monitor service user progress and communicate updates to relevant team members.
* Provide administrative support, including scheduling, documentation, and correspondence related to service user care plans.
Skills
* Proven office experience with excellent organisational skills.
* Strong communication skills, both written and verbal, with an empathetic approach towards service users and their families.
* Ability to multitask effectively in a fast-paced environment whilst maintaining attention to detail.
* Proficient in using office software programmes such as Microsoft Office or similar applications.
* Ability to work collaboratively within a multidisciplinary team.
