We are in the process of developing a transition plan for an individual to move back to North Yorkshire. This is a great opportunity to be part of developing this new package of support. We provide person centred individualised care with a focus on the model of Positive Behavioural Support (PBS)! As a Support Worker, you will be directly providing support to the individual and helping to develop their skills, build their home, be a part of their community and most of all improve their quality of life.
