The post holder is required to work within an MDT focusing on the older person living in the community who suffer with a wide range of mental health problems. Main duties of the job The post holder will be responsible for assessing client’s needs, planning and implementing the care package alongside other professionals within the team. Post holder will provide education, knowledge, guidance and support to student nurses and unqualified staff in the team and provide support to clients and their families. The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply. Detailed Job Description And Main Responsibilities In conjunction with other team members, to take on the role of care coordinator for a designated caseload of clients with both organic and functional mental health problems. To be responsible, for the comprehensive assessment, planning, implementation of programmes of care, providing each client with an individual holistic package of care. To ensure Mental Health Measure and care management protocols are adhered to and standards are met. As care coordinator, responsibility for making evidence based assessment of client’s physical, psychological, spiritual and social needs taking into account and respecting ethnicity, for a defined caseload.
