We are looking for a Specialist nurse for Children with Medical Complexities to join our team. This post offers opportunities to develop strategy, service development and improvement and to deliver joined up, high-skilled care to children with medical complexities and their families.
The successful candidate will work with specialist and clinical teams as well as the Roald Dahl Marvellous Children's Charity to develop and deliver a well organised and responsive service. Access to a wide variety of training opportunities from both the NNUH and the Roald Dahl Marvellous Children’s Charity (RDMCC) will provide a supportive community of other CMC Nurses.

Main duties / Responsibilities
The successful candidate will work with clinical and specialty teams at the NNUH to identify and support a caseload of patients with high medical needs, providing both highly skilled clinical care and holistic care to patients and their families through admission planning, acute/elective presentations to the NNUH and through complex discharge coordination. The post holder will have access to training and support from the Roald Dahl Marvellous Children’s Charity to support learning and the families on the caseload.
As this is a new service, we are seeking someone with experience in change management and service improvement, capable of developing the role and presenting progress to the NNUHFT and to the RDMCC. The successful candidate will be self–motivated with good organisational skills and will be prepared to present and feedback to both the NNUH and RDMCC to progress the service.
Core focus includes ensuring patient safety, delivering patient-focused care, and maintaining excellent communication to provide high-quality care.

Job description and responsibilities
The role involves working with professionals from diverse backgrounds, patients and their families to deliver holistic, joined up care. Responsibilities include autonomous advanced clinical practice, managing a caseload, and coordinating multidisciplinary assessments to address physical, psychological, emotional and spiritual needs of the child and family.
* Operate autonomously at an advanced level of clinical practice, providing research-based expert care and managing a caseload effectively
* Collaborate with MDTs, CNSs, clinical teams, Consultants, therapists and community nursing teams to improve patient flow, capacity and continuity of care
* Accept referrals from medical/nursing staff within and external to the Trust
* Ensure patients are seen within relevant timescales and targets
* Coordinate multidisciplinary assessments and develop strategies to meet needs of the CYP and family
* Respond to telephone enquiries, assess patients via telephone, coordinate admissions or appointment changes
* Prepare and participate in patient reviews and child protection case conferences; provide reports as required
* Administer medications as per Patient Group Directions or Trust prescribing policy for nurse prescribers
* Maintain professional relationships with patients, relatives and the multidisciplinary team
* Make judgments for patient management and initiate referrals to multidisciplinary specialists as appropriate
* Contribute to service developments and nurse-led initiatives in liaison with leadership and Roald Dahl Charity
* Contribute to divisional, Trust and corporate objectives and service plans in collaboration with the Roald Dahl Charity

Person specification
Qualifications (Essential):
* Registered Nurse on the appropriate part of the NMC register (RN Child/RSCN)
* Postgraduate qualification relevant to the specialism or equivalent
Experience (Essential):
* Extensive post-registration experience, with significant experience in the specialist area
Skills (Essential):
* Demonstrable leadership skills
* Effective communicator
