Band 5 Pharmacy Technician

Job Overview:
As a Band 5 Clinical Pharmacy Technician you will work within the Pharmacy Department at this busy NHS Trust, supporting delivery of safe, effective, and patient-centred medicines optimisation services. You will work closely with pharmacists and the wider multidisciplinary team to provide high-quality clinical pharmacy support across inpatient areas, contributing to improved patient outcomes and the efficient use of medicines.
Key Responsibilities:
1. Undertake medicines reconciliation for patients on admission, transfer and discharge
2. Support Ward-Based clinical pharmacy services alongside pharmacists
3. Work collaboratively with the multidisciplinary team to optimise medicines use
4. Provide accurate medicines-related information to patients and healthcare professionals
5. Ensure compliance with local policies, national guidance, and NHS Standards
6. Contribute to service development, audit, and quality improvement initiatives
7. Maintain accurate clinical and pharmacy documentation

9. CPT Qualification
