Harlow South PCN is looking for an innovative and highly motivated person to join its team as a Care Coordinator.
Care Coordinators play a key role within a PCN to support patients to access the care and services they need. As part of the PCN team, Care Coordinators support proactive healthcare by contacting patients and helping them to engage with important preventative services such as cancer screening programmes, immunisations and chronic disease monitoring. Providing coordination and navigation of care and support across health and care services. Care Coordinators also support patients to become more digitally enabled.
Main duties of the job
* Proactively identify and work with a cohort of patients to support their personalised care requirements.
* Provide coordination and navigation support using digital tools to help patients access appropriate services.
* Develop and maintain personalised care and support plans based on an individuals needs and what matters to them.
* Promote preventative health care and continuity of care.
* Act as a key point of contact for Safeguarding and follow up on related actions.
* Provide support and guidance to patients, families, and carers, ensuring a patient‑focused approach.
* Work collaboratively with internal teams and external services to maintain efficient care delivery.
* Plan and prioritise own workload, using initiative while contributing effectively to the team.
* Maintain strong IT skills, including Microsoft Office, Outlook, and Excel, to manage administrative tasks.
* Record patient interventions on relevant electronic database systems (e.g. System One).
* Be customer‑focused when representing the service.
* Excellent customer service skills and the ability to put the needs of patients first.
* Computer literacy, including a working knowledge of relevant programs, such as Word and Excel.
* Ability to function well in a team to provide effective administrative support.
* Excellent organisational skills to ensure that an office runs smoothly.
* Ability to maintain strict confidentiality and deal with the public in a sensitive way.
* Good spelling and grammar for typing clinical and administrative letters and other correspondence.
* Knowledge of medical terminology to accurately type out medical reports and other documents.

Harlow South PCN works for Lister Medical Centre, Hamilton Practice and The Ross Practice. Serving a population of over 40,000 patients. Delivering services on behalf of practices and running enhanced access. Harlow South PCN is based at Lister House Medical Centre.
Qualifications
* GCSE grade A to C in English and Maths
* Candidates must be able to work cohesively as part of a team
* The ideal candidate must be confident, a quick learner, possess excellent communication and organisational skills, have a polite telephone manner along with the ability to deal with sensitive/confidential issues appropriately whilst remaining calm under pressure.
* Previous experience working in a medical environment
