This is a great opportunity to join our Triage Hub based at Middleton Health Centre. We are seeking experienced and motivated senior clinicians with either a nursing or Allied Health Professional (AHP) qualification, who have a keen interest and/or experience in clinical triage.
You will be responsible for the triage and prioritisation of all new referrals, as well as the prioritisation of planned and unplanned care for new and existing patients.
The role requires expert clinical knowledge and judgement to support decision-making. You will be expected to spend time in clinical practice to enhance and support this.
This is an office-based role, but we offer flexible working arrangements to support your work-life balance. The service operates from 7am to 10pm, seven days a week.
Main duties of the job
You will prioritise planned and unplanned care based on urgency, supporting patients through their care journey. You will work closely with primary and secondary care, as well as local third sector and voluntary organisations.
The Triage role aims to ensure patients receive care in the most appropriate setting, by the most suitable team, at the right time. You will conduct virtual consultations with staff, patients, carers, and other professionals.
You will adhere to professional and regulatory standards, ensuring your clinical practice is evidence-based and grounded in current knowledge. This is an exciting role within a dynamic service, and we welcome your application.
