Social Worker | Full time | Urgent Care and Short Term Support | Central, Lancashire
Job summary: Lancashire County Council is seeking a Social Worker (Full time) for Urgent Care and Short Term Support in Central, Lancashire. Salary £36,363 - £40,778 per annum. Permanent contract, 37 hours per week.
Responsibilities
* Provide holistic, face-to-face Care Act Assessments, reviews and support planning using a strengths-based approach to help people achieve independence and live in their chosen home.
* Work across Lancashire as part of the Urgent Care Acute and Prisons Service; support hospital discharges, work with the wider multi-disciplinary team to prevent hospital admissions, and utilise the Discharge to Assess model.
* Coordinate with the team to ensure assessments and support planning enable optimum independence and living at home.
Requirements
* Social Work England registration or eligibility and application to register with SWE.
* Demonstrated empathy and commitment to making a difference; ability to work creatively to support people in achieving outcomes.
* Access to a car for work duties and valid business insurance; driving is essential or consider reasonable adjustments if unable to drive due to disability.
