Job Overview
We are seeking a motivated Band 6 Physiotherapist to join our dynamic Community Therapy Team. This role involves delivering high-quality physiotherapy services to housebound patients across the Baywide locality, encompassing community rehabilitation, crisis response, and intermediate care. You will work autonomously and as part of a multidisciplinary team, contributing to the South Devon Intermediate Care weekend and late duty rotas.

Responsibilities
* Conduct advanced physiotherapeutic assessments for patients with diverse physical, psychological, cognitive, and behavioural conditions.
* Develop and implement personalised treatment plans, including manual therapy, exercise programmes, and patient education.
* Provide mentorship to junior staff, students, and support workers.
* Collaborate with multidisciplinary teams to deliver integrated care.
* Participate in service development and quality improvement initiatives.
* Contribute to weekend and late duty rotas as required.

Requirements
* Degree or diploma in Physiotherapy.
* HCPC registration.
* Experience in NHS settings, including rotations in orthopaedics, elderly care, medical, surgical, neurological, and musculoskeletal conditions.
* Evidence of continuous professional development (CPD).
* Experience in supervising students and support staff.
* Ability to work independently and manage a varied caseload.
* Full UK driving licence and access to a vehicle for community visits.
