Post Diagnostic Link Worker Fixed Term 1 Year Job Purpose The Dementia Link Worker will initially be based in the Older People’s Acute Mental Health Day Services (OPADS) in West Lothian and will be part of the range of services attached to the OPCMHT. The Dementia Link Worker will deliver a minimum of one year’s Post Diagnostic Support to people newly diagnosed with dementia and will enhance the support already available from the Older People’s Acute Mental Health Day Services, Community Psychiatric Nursing Team and the Dementia Advisor. The Worker will provide advice, information and support to people with dementia and their carers who are in the process of, or have been diagnosed with dementia. The worker will be using the 5 Pillar Model of Post Diagnostic Support. This post requires the post holder to have a PVG Scheme membership/record. If the post holder is not a current PVG member for the required regulatory group (i.e. child and/or adult) then an application will need to be made to Disclosure Scotland and deemed satisfactory before they can begin in post. Due to legislative changes from 1 April 2025, this post may require a different level of criminal records check done than is currently the case. If the post is assessed as a "regulated role", your appointment will be subject to joining the Protecting Vulnerable Groups (PVG) Scheme.
